Probably the serial was only checked from the cd cover, other PlayStation the Best/PSone Books Edition dumps might have the same issue, maybe Grandia (J) and others should be checked for this...

i'd always post release serial
in Japan exe name stay the same most of the time even if cd itself is modified
Internal serial should be included too, just like in Tales of Phantasy and Tales of Eternia there is a Original edition that matches...

I know that some cover scans will change the serial on them even if the disc's internal serial is unchanged. Is this the case in all of these situations?

yes, it's a serial of a release, but then how otherwise to identify and cd? ahhh i don't understand this, why is exe name important? 
i thought that we dump everything we can and then if different releases match they will stack up in a same record in db with multiple serials and versions. so initial serial will eventually arrive there but only when that exact cd is checked. and exe name i think can be wrong also sometimes - it's rare but can be - like typo, or cd2 can have it from cd1, or it can be psx.exe.
