I did one and I think Haldrie did one too. (Thief for me, and I think Tomb Raider III for him)
dump the data tracks as normal and the audio as normal. The cue sheet can handle it fine.

I have this game too, and I dumped it.
Problem is we can't agree on part of the pregap. It should be 5 seconds, but it contains 2 data parts and 2 audio parts. The pregap can be retrieved by dumping with cdtoimg d8 mod on a plextor. You can then use descramble_cdda to unscrambled the 2nd data part, after pulling it out of the image with a hex editor. Problem arises because the disc was mastered bad, and the 1st data part in the pregap has some audio in it. ir0b0t and I think it should be descrambled, F1REB4LL however thinks it should be left scrambled. Either way the pregap needs to be attached to track04 by hand with a hex editor.
I started to make a guide for CDLock, but this disc threw me off because it differs from the others I have. They have a 3 second pregap, 1 second audio, 2 seconds data and no offsetting. So the data can be descrambled properly without any issues on sector alignment.
You can get the offset of the data by multiplying the sector start address EAC gives for track04 and multiplying it by 2352, then you calculate the size of the pregap, and multiply that by 2352, and subtract the two results to get the base offset. Then you need to add the combined offset (in bytes, which is samples*4) to that to get the real offset in the image for it.
This also points out where any imaging tool will fail to image such a disc, as the combined offset is not compensated for when reading the image, and there will be combined offset bytes missing from the last data track. The firmware in the drive when reading the track with Isobuster will search for the sector id sequence and compensate automatically for the offset. You can verify that by making sure the track's size is a multiple of 2352.

The sub is all screwed, 200 sectors are totally missing (?) at the end of the 3rd track, then go 100 sectors with FFs in the P channel, but belong to the 3rd track according to the Q channel, so the gap between the 3rd and 4th sectors is 00:02:00. I repeat, the sub is screwed, so it's hard to tell something.

You forgot to check the sub reading in the CloneCD profile, so this .sub is generated, not from the disc.

Weird, this one shows 6:00 last gap, consists of data sectors only. Anyway, wait for the Nexy's dump first.

Mine is USA also, Ammo Pack edition, but it's version 1.0... in this case maybe is the same as original.
