I have this same disc. I can fill in the missing info:
Text on disc front: "FOR USE ONLY IN USA, CANADA, MEXICO AND LATIN AMERICA RVL-R8PE-USA MODEL NO. RVL-006(USA)"
Text on disc back: "IFPI LL08 C07F1504 RVL-R8PE-0A-1 USA S0"
Barcode: 0 45496 90015 1 00101
When submitting your dump, you should also transcribe the text on the back and front of your disc and the barcode on the back of your game box.
