It's not Need for Speed: Underground, it's just Need for Speed Underground. That's what it says on the case, and on the website: http://www.ea.com/official/nfs/underground/us/ (clearer on the FAQ).

Disc: Need for Speed Underground (Disc 2) [Disc 1 has no protection]
Protection: SafeDisc v2.90.040

I've used DDump on this disc, ripped it three times, and got the same image and logfile each time. However, there are a number of bad sectors outside of the 1-10,000 range specified in the SafeDisc thread. Otherwise everything else seems fine. Is there anything I need to do before submitting it to the database?

Also, this is the first time I've submitted a SafeDisc rip - is there anything else I should know besides what was described in the thread?

Edit: Something else that occurred to me: should the .exe date be the date of the game executable or the setup executable?
